After you have completed the application, please return it to Our Lady Academy with all other required forms and the non-refundable registration fee ($200 per family). This begins the application process. Applications are processed on a 'first received basis'.

Please send this form to your child's current school (or grading service if home schooling) requesting that a
copy of all transcripts and test scores be sent to Our Lady Academy.

Once the application and all records have been received, Our Lady Academy will review them. At that time,
you will be contacted to participate in an interview including both parents and student(s).

As part of the interview process, your child may be assessed. This assessment helps us understand your
child's academic skills and appropriate grade placement.

Following the interview, assessment and record appraisal, you will be notified either verbally or in writing of
your child’s acceptance status to Our Lady Academy.
